For new patients, once you have made your initial appointment, you may pre-register by downloading, printing, and completing the new patient packet below. Doing so will help speed up the new patient registration process. Please be sure to bring the forms with you to your first visit.

Established patients can update forms online through our patient portal. If you need help accessing the portal, please call the office, and we’ll be happy to help.

Insurance / Payment / Billing
At Elite SkinMD, we currently participate with:

If you have out-of-network benefits, we will be happy to file a claim with the insurance company on your behalf. Please contact your insurance company or you may call our office at (908) 787-8088 to find out if we accept your insurance plan.
If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible, you will need to pay at the time of your visit. For your convenience we accept cash, checks, Visa, MasterCard, Discover, and American Express. There is a $50 fee on all returned checks.
